    OK latest update:
I got everything back on track, I haven't got any freezing or driver errors for almost about a week now, longest ever. What did I do?
I installed the latest (october) drivers for GPU.
So, it was software all along. No voltages, temps, nothing. Just software. So frustrating.

What I can't make sense is, why did 338 drivers work on my old computer, but not on the new one? It's the same GPU... just different computer (hardware). Oh well!
    Thanks everyone for help.

If you did not try yet here's what helped out my problems

only place 1 stick of ram into the board (just for testing)
I see your baseclock was above 100 (100.02) put this to 99.76 .yep i know sounds strange but its a known bug in gigabyte biosses

just try it and see if it helps

thanks for your advice, but as I already stated, the latest drivers sorted things out and I have no issues now.
