Many factors some chips won't run stable at certain frequency no matter what voltage you through in.
are you sure is 100% stable?
2x8pins wasn't enough nvidia? also smaller the connectors more chances of melting.
why not driver level to lock fps?

I only trust Prime95, and OCCT when ever need to test stability. so if you manage these two, you have most probably stable oc
if it fitted directly to the MB without back plate then its not advisable.

Its not a fault,or it can not run v3 specs, it was not tested for gen3 when it launched.
on launch it was not certified. my second pc 3930k chip running pcie v3 without any problem through registry 8GT implementation.

sandy bridge-e fully support pci-e v3.
all depend on chip. use latest prime95 for atleast 3 hours. ur cooler wont handle much.
