Any recommendations regarding performance? The game runs at high fps for me (after running in borderless fullscreen, fullscreen itself gives the 60fps lock), yet both modes feel stuttery.
When I run MSI AB, the frametimes jump between 10-20ms, which doesn't make much sense.

    Did you try this: https://www.pcgamingwiki.com/wiki/Mafia:_Definitive_Edition#Issues_fixed

Play it fullscreen (not borderless) and force vsync from nvcp, disabling the one ingame. I have no stuttering at all this way.

Thanks, it does indeed! I'm back at 60fps but thgame feels much more fluent now. I'll need to test it a little longer but I did the race early in the game and it looked very promising!

Am I the only who finds the game blurry? The lighting is pretty good and indoors as well. But outside looks blurry. I also find the LOD at medium distances to be low and the trees and foliage look low resolution. Everything is on high except motion blur which I turn off in every game. Was expecting more to be honest judging by the comments. I actually find RIS to be meaningful for once and helps a bit.

Also what's up with the wheels animations?

The wheels problem is TAA. It's ghosting from the previous frames. You can try disabling it (it's called "Antialiasing" in the graphics options), but the jaggies will go up the roof.
About the blurriness, you will have to use CAS Sharpening set to about 90%. It will look a lot better.

Never cared about these mafia games, but for those who do, this might be interesting. Former mafia capo Michael Franzese gives his impressions of Mafia 2:
