I only recently got this problem after installing 182.06 drivers. I have setup Low-level fan control on Auto with the necessary values and it has been working for me for about 1.5 years. Now whenever the PC reboots it goes back to "Fixed" even though it is set to automatically start with the "Auto" settings. It does not matter if I roll back to Nvidia 181.22 or any earlier version of Rivatuner (tried 2.21 and 2.11). What recently changed: Got a new screen, so now I am running dual screen setup, installed new 182.06 drivers, installed Rivatuner 2.23. Running on Windows XP.
Got a work-around. Leave the auto settings as they were and save a Custom Profile. Then go to scheduler and set an event to fire the Custom Profile when the GPU temp hits 70°C. Works like a charm.
