I noticed long ago (No matter what driver I use) that under the SLi tab in the Nvidia control panel, it says my monitor is connected to the 2nd 980Ti card when in fact its NOT. Why is this, is it hurting performance, and can i fix this if this is in fact "an issue"?? HEre is a picture of what im talking about:

Very strange , just checked mine and it shows that im connected to card (1) with displayport which is correct, so i don't know why yours says it connected to (2)

What would happen if you did indeed connect your monitor to your second card ? give it ago maybe ,,,

i have no screen until next reboot if i plug my monitor into 2nd card, and upon 2nd reboot, sli is disabled automatically and then screen works... I re-enabled sli, plugged monitor back into my topcard, and same thing.... says monitor is plugged into 2nd card... so weird

Something surely strange is going on with your system then.
From the screenshot of your Configure SLI, Surround, PhysX settings, I also noticed that PhysX is auto-detected to run on the primary GPU 1 rather than the secondary GPU 2.

For me, PhysX is always auto-detected to run on the bottom card (GPU 2).
Perhaps these issues are somehow related?

BTW did you try another DisplayPort cable and/or SLI bridge?

Uninstall your driver in safe mode using DDU, then reinstall and report back.
