*PROBLEM* Extremely Frustrated - 2D Surround & Accessory Display Selection

First, my System;
Windows 7 Ultimate 64-bit, all updates.
EVGA GTX680 OC Signature + Backplate, DRIVER: 301.42 -or- 304.48
ASUS Sabertooth Z77
Intel i7-3770k
G.Skill Ripjaws 16GB DDR3-1600 (4x4)
OCZ V3-MI 120GB

Displays:
3x Dell S2409W LCD (each have 1x HDMI, 1x VGA, 1x DVI)
1x Insignia LED LCD (has 3x HDMI, 1x VGA, composite, coax)

Here is a pic of the setup with surround disabled and monitors arranged appropriately.



No matter how I connect these displays, the Insignia always tries to work itself into the surround configuration once enabled.



Surround works fine when the Insignia isn't connected.



According to this document any of the three connectors on a single GPU can act as surround displays, and the fourth connected display becomes the accessory display.

This is extremely frustrating, someone please help!

 

Right click on desktop -> Screen Resolution.
Enable the 3 displays for surround, disabling your accessory.
Put them in the correct order as they are on the desk (123).
Hit Ok

Now go back to nVidia Control Panel and try to enable surround + accessory.

 

I tried this during my troubleshooting steps last night, but figured I'd give it a shot again... same result.

 

Finally got it... I used an HDMI>DVI adapter on the monitor-side of the HDMI cable that was feeding my 3rd S2409W.



I will update a thread with a quick Visio showing how I have them connected.

 

Here is a diagram to better explain how I connected everything.

For the connection from the DisplayPort on the card to the HDMI port on the Insignia TV, I am using a 15' DisplayPort to HDMI cable.
